- Enhanced CRB clearance
- Passport & proof of eligibility to work in the UK
- 2 Clinical references
- Evidence of professional qualifications
- CV
- National Insurance Number
- Immunisation Record (Hep B) and Hep C for certain Trusts
- Proof of current UK work experience
